is committed to giving area residents needed care in the
comfortable surroundings of home. Our staff includes:
skilled nurses; physical, occupational, speech therapists;
social workers; nurse aides; and a dietitian. We stay in
close touch with your physician to plan your care, note your
progress and make changes as needed in the care plan.

Hospital Home Health
services are available to patients who:

Require
skilled nursing care, physical, occupational or speech
therapy.

Were
recently discharged from a hospital or other healthcare
facility and require continuing care, monitoring or
teaching.

Have
difficulty getting to the doctor’s office and require
skilled care.

Need help to
regain independence in walking, bathing or talking.
Our highly
qualified home health nurses are available to:

Plan your
nursing care with your physician’s guidance.
-
Provide
post-operative care and monitoring for surgeries such
as: joint replacements, abdominal surgery, open heart
surgery and many more.

Provide care
and treatment for wounds.
-
Provide
patient and family teaching for: medications, present or
on-going illness, wound care, glucose monitoring and
insulin administration, ostomy care and more.

Our nurses
can: administer IV medications such as antibiotics,
obtain lab specimens, change foley catheters, provide
ostomy care, etc.
Our physical
therapists help you and your family members to:

Provide an
individually designed plan of care to return functional
abilities to optimal level.

Plan and teach
home exercises to recover movement or regain muscle strength
and function.

Select and
teach activities to restore physical function.

Carry out
physician orders for treatment and report progress or
changes needed in the plan of care.

obtain adaptive home equipment or other supportive devices.
- Teach adaptive methods of
eating, dressing, and other daily activities to improve
independence.
- Design, make, and fit
orthotics and self-help
devices.

help speech-language communication skills.
- Assist those with voice
disorders to develop proper vocal and respiratory control.
- Assist those with
swallowing difficulties.
* Services
provided as qualified personnel are available
- Provides nutritional
evaluation and teaching.
- Provides teaching for
special diets such as: diabetic, healthy heart, renal, high
protein and low sodium.
* Services
provided as qualified personnel are available
- assess social and
emotional factors related to an illness and its treatment
- Assess need for other
services such as: financial and community resources.
* Services

Our
aides support the services of the care professionals as they:

Help with
personal care such as: bathing, skin and hair care, and
bed linen changes.

Assist with
exercises which have been prescribed.

Supervise
and support the tasks of daily living.

Encourage
self-help whenever possible.
Both Medicare
and Medicaid cover the complete cost of Home Health
services. Additionally, many private insurance plans have a
Home Health benefit.

your physician has recommended home care services, or you
feel you are in need of our services, please call us at
(419) 947-9127 during office hours Monday through Friday, 8
a.m.-4:30 p.m. After office hours, or on the weekend, please
contact the Home Health on-call nurse at Morrow County
Hospital –
(419) 946-5015.
