Visit Information

If you are a new patient, please bring your completed New Patient Packet with you to your first appointment. If you are a returning patient, please complete the Patient Information Update Form to tell us what, if anything, has changed.

To care for you as efficiently as possible, please bring the following items with you to your first new patient visit:

  • Photo ID
  • Insurance card
  • Medicare pharmacy card
  • Co-payment
  • All medications in original containers, including over-the-counter medications
  • Completed patient forms
  • Prior medical records, if available, and test results or imaging results that may have been requested

New patients should arrive 15 minutes early to your scheduled appointment if you have your completed New Patient Packet with you. If you have not completed this, please arrive 30 minutes early to give you time to complete the form.

Returning patients should arrive 15 minutes early to your scheduled appointment to complete your Patient Update Form.

Patient After Office Hours Instructions
Medication Refills

It is important that you keep track of your medication refills and call the office during our regular office hours to refill medications. If you have not been in for an office visit within the last six months, you will need to make an appointment to get these refilled. Depending on the prescription, we may call in a prescription for enough medication to last until your next appointment. Allow our office one to two business days to get these refilled.


Appointments at MCH Primary Care Northfield will ONLY be scheduled during our regular office hours. If you are unable to keep your appointment, we kindly ask that you give us a 24- to 48-hour notice. However, if you wish to schedule or reschedule an appointment, we ask that you call back during our regular office hours.

Illness and Medical Emergency

MCH Primary Care Northfield does not prescribe any medication after office hours. If you are sick and/or feel that you are experiencing a medical emergency, please call 9-1-1 or go to the nearest emergency room or urgent care for treatment.
