Public Records Notice

Morrow County Hospital strictly adheres to Ohio’s Public Records Act of the Ohio Revised Code. The following is a summary of your rights regarding the request of public records.

  • Ohio Revised Code 149.43 grants any person the right to inspect a public record at any reasonable time, and, upon request, to receive copies of a public record, at cost, and within a reasonable period of time. Public records do not include patient medical records or other records that are exempt.
  • No specific language is required to make a request of public records; however, the requester must at least identify the records requested with sufficient clarity to allow Morrow County Hospital to identify, retrieve and review the records.
  • The request does not need to be in writing and the requester does not have to provide his or her identity or the intended use of the requested public record.
  • Public records are to be available during regular business hours and must be made available within a reasonable period of time.
  • If Morrow County Hospital denies the request of any public records or any part of a public record, such denial must include a legal explanation.
  • If portions of a record are both public and exempt, the exempt portions are to be redacted and the rest to be released with a supporting explanation regarding the redacted portions.
  • Those seeking public records will be charged ten cents ($.10) per page. The charge for records downloaded to compact disc is one dollar ($1) per disc. If making copies; however, records transmitted electronically, if feasible, are free to the requester.

Discrimination is Against the Law

Morrow County Hospital complies with applicable federal civil rights laws and does not discriminate, exclude people, or treat them differently because of race, color, ethnicity, religion, culture, language, national origin, age, disability, socioeconomic status, sex, sexual orientation, or gender identity or expression in its health programs and activities.

Morrow County Hospital provides free aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, other formats).

Morrow County Hospital also provides free language services to people whose primary language is not English, such as qualified interpreters. If you need these services, please call 1-614-899-6666.

If you believe that Morrow County Hospital has failed to provide these services or discriminated against you, you can file a grievance by sending a written letter to Morrow County Hospital (651 West Marion Road, Mt. Gilead, Ohio 43338), or by calling the hospital’s Compliance Office – (419) 946.5015.
